To Register please gather the following documents: 

  • Guardians Identification 
  • Student’s Birth Certificate 
  • Copy of Student’s Immunizations 
  • 2 Proofs of Residency (Deed, mortgage, lease agreement, tax statement, voter registration, verifiable form of income displaying address, court order, or utility bill dated with the past 30 days 

 

Once you have the documents, please contact your students building registration person to set up an appointment to complete the registration application.

 

Elementary ( Pre-K – 6th Grade)

  • Copy of Student’s IEP or 504 Plan  
  • Copy of Student’s Latest Report Card 

 

Pre-K 

Please note students registering for Pre-k must be 4 years old on or before the first day of the current school year.  Pre-k is a dual enrollment with head start so additional forms need to be completed, and proof of income must be provided. 

 

Kindergarten  

Students registering for Kindergarten must be 5 years old on or before the first day of the current school year. 

Students currently enrolled at the Carbondale Area Pre-k program do not need to register for Kindergarten.  

 

 

IMMUNIZATION REQUIREMENTS 

 

PRE-K 

Diphtheria and Tetanus Toxoid – Initial series (3 doses) 

Polio (3 doses) 

MMR (1 dose) 

Hepatitis B (3 doses) 

Varicella Vaccine 

Chickenpox Vaccine (1 dose or diagnosis of disease or laboratory testing) 

 

KINDERGARTEN

Polio (3 doses) 

 MMR (2 doses) 

Hepatitis B (3 doses)

Safe2Say Something is a youth violence prevention program run by the Pennsylvania Office of Attorney General. The program teaches youth and adults how to recognize warning signs and signals, especially within social media, from individuals who may be a threat to themselves or others and to “say something” BEFORE it is too late.
